Your First Visit

Please arrive about 10 minutes early so that you have enough time to complete any necessary forms. You can either complete a New Patient Form* and Privacy Form when you arrive at your first appointment, or download them below, complete them and bring them with you. 

New Patient Form

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

Initial Examination

To start, your dentist will review your medical and dental history and complete a comprehensive dental exam. This exam will include a thorough inspection of your bite, jaw, teeth and gums, as well as an oral cancer screening and any x-rays that may be needed. 

Treatment Plan

Your dentist will develop a long-term dental treatment plan for you. Any questions you may have about your oral health and treatment options can also be addressed. 

Insurance & Financing

At North Delta Dental Professionals, we'll help you navigate our options for payment, direct billing to insurance companies and financing so you can focus on getting the dental care you need.

• Direct Billing to Insurance

  For your convenience, we offer direct billing for most insurance plans. For your convenience, we can submit your insurance claim electronically. For you, this means faster reimbursement, no signatures or paper forms needed. 

  Our administrative team can help you understand your insurance plan better, and cal also send estimates to your insurance company before starting any major treatments. 

  Please bring your insurance plan information to your appointment.

• Payment Methods

  You are responsible for any amounts not covered by insurance. These must be paid at your appointment time. 

  We accept payment by cash, cheque, debit or credit card.

• How can I book an appointment?

  To book an appointment at North Delta Dental Professionals, please contact our office during our regular business hours.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please call us. Please notify us of any change of plans as soon as you can, so we can offer the space in our schedule to another patient.

• Do you offer appointment reminders?

  In the days before your appointment, you will receive automated phone appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to notify us.

• What are your policies regarding missed appointments?

  We always ask that patients arrive a few minutes before their appointment time to check in, and to let us know as soon as possible if they need to cancel or reschedule an appointment. 

  If an appointment is missed, this means we are unable to arrange another appointment for a patient in that timeslot. 

  A missed appointment without notification may incur a fee. 

• Do you provide services in any language other than English?

  Yes, our office is also able to provide dental services in Punjabi.

  If you have a language preference, please advise us when booking your appointment.

• Does your clinic have on-site parking?

  Yes, we have free on-site parking available for all our patients. You can also easily reach us via public transit.
